OBJECTIVE/OVERVIEW: CUHCC’s mission is to seek health equity in our community by advancing the well-being of diverse people. The clinic is known for providing high quality care to diverse patients and is moving forward in improving timely access to care for patients. This position directs and oversees and supervises the clinical work of medical and psychiatry patient care nursing staff and works to maximize the clinic’s flow and efficiency. This position reports to the Clinic Manager.
ESSENTIAL FUNCTIONS: Patient Care & Care Coordination: 70% - Administer care/treatment and care coordination to patients as ordered by medical and mental health providers. - Participate and work together with team members to ensure that patients receive whole-person care. - Conduct patient and family education related to preventive care, mental health care, chronic care and condition management as appropriate; connect patients to resources when their conditions are exacerbated. - Manage a panel of patients, many with multiple medical and mental health diagnoses, and other patient populations as assigned. - Develop patient care plans based on risk and overall patient care complexity. - Work with care managers to implement the patient’s care plan. - Communicate with providers, care coordination team members, and other support staff through EHR, in person, and other means. - Communicate significant changes in patient status to provider and care team. - Facilitate medication refills and provide medication education for patients as needed. - Identify and connect patients to community resources that will assist them in achieving good health outcomes. - Follow-up with patients if they have not scheduled their appointments. Check lab/path results from Fairview and if needed call Fairview for results. - Follow-up with no-show patients that have not rescheduled. - Work with interpreters and/or PSRs to call and schedule non-English speaking patients accordingly. - Orient residents and nursing students as needed. - Provide walk-in and phone triage. - Respond to flags, Emergency Department/Inpatient Hospitalization summaries, refill requests.
SUPERVISE NURSING STAFF: 25% - Provide supervision to all Registered Nurses in both Medical and Behavioral Health areas. - Hire, train and orient new nursing staff. - Encourage team building, staff education and clinic flow process improvement with nursing team members. - Delegate tasks to nursing team members as appropriate. - Construct a monthly schedule for nursing staff ensuring adequate coverage for vacations, planned sick leave, etc. - Review and approve time cards for nursing staff members. - Create daily work assignments for nursing staff. - Take ill calls, calling temporary agency for staffing needs. - Be the point person for temporary nursing staff, collect and route temp staff timecards and provide detailed direction to temporary staff members. - Attend and participate in regular management meetings as directed by Clinic Manager and/or COO - Develop written training protocols and competency assessments for nursing staff. - Provide staff coaching, probationary and annual assessments and other staff recognition and/or discipline. - Maintain personnel files for each nursing staff member. - Execute Performance Reviews and manage performance concerns to all supervisees (RNs)
OTHER DUTIES AS ASSIGNED: 5% - Attend all-staff meetings, weekly care coordination meetings, weekly nursing meetings, and daily huddles. - Other duties as assigned.
NON-ESSENTIAL FUNCTIONS: NONE.
REQUIRED COMPETENCIES/SKILLS: a. Nursing assessment aptitude and problem solving skills b. Strong time management and organizational skills c. Knowledge of regulations, policies and procedures for nursing in the primary care setting. d. Proficiency in utilizing an electronic medical record, registries or databases to monitor patient outcomes. Proficiency in electronic tools with the EMR to communicate with e. Must be able to lift 30 lbs. f. Willingness to work 4-8 hour shifts between the hours of 7 a.m. and 8:00 p.m. Monday through Friday and some Saturdays from 9:00 a.m. to 1:00 p.m. g. Must be able to work in a fast-paced, rapidly changing environment that requires sitting, standing, and walking, with frequently changing priorities. Must be able to be flexible and multitask. h. High-level communications skills to work with patients, staff, providers, and outside agencies. i. Excellent customer service skills. j. Willingness and skill in working with patients and staff from diverse cultures, languages, and backgrounds. k. Capacity to work on a team and make positive contributions to the team.

REQUIRED QUALIFICATIONS: 1. Bachelor’s degree in Nursing from an accredited school of nursing with at least two years of experience or a combination of related education and work experience to equal six years. 2. Current licensure as a Registered Nurse in State of Minnesota. 3. Experience in staff supervision. 4. CPR certified. 5. Experience in managing clinical care using an electronic health record.
PREFERRED QUALIFICATIONS: 1. Two to three years of experience in providing care coordination or skilled nursing. 2. Proficient in one of CUHCC’s dominant languages: Spanish, Somali, Vietnamese, Hmong, Lao. 3. Experience or certification in mental health nursing. 4. Experience in providing care coordination to a complex, diverse patient population.
