Job Summary:

The overall goal of this position is to improve organizational clinical/operational outcomes for spine related conditions and procedures. The Coordinator coordinates, delegates, provides, and evaluates care of patients, including planning and delivering patient-focused care in partnership with interdisciplinary teams, the patient, and family. The Coordinator helps individuals with spinal conditions navigate the medical system and receive timely and appropriate evaluation, treatment, and education services. The patient needs may range from minimal to complex and may occur in the inpatient or ambulatory arena. The Coordinator is responsible for facilitating a patient and physician friendly experience within the AdventHealth Spine Care Program. The Coordinator assists in planning and conducting community outreach activities, including screenings. The Coordinator assists patients in accessing appropriate resources, as needed. Coordinator may facilitate patient support and/or education groups and strives continually for innovative and creative strategies to affect and improve patient quality of life. Coordinator will gather, record and evaluate program data and to use this data in establishing and evaluating program goals. Coordinator will facilitate, with physician, administrative, and nursing leader(s), an interdisciplinary team to support clinical/operational goals, establish growth/volume/market goals, implement evidence-based protocol, monitor adherence to established patient pathways, etc. The coordinator actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all. Knowledge, Skills, Education, & Experience Required:

• Skilled in providing age appropriate spine care education and patient care.
• Team-centered leadership and facilitation skills; ability to interact appropriately with teams to facilitate performance improvement; ability to interact with patients/families and Spine Care Program Team in regard to care management interventions and illness adjustment issues.
• Strong organizational skills
• Strong communication and customer relation skills; ability to interact patiently with patients with chronic pain and/or illness
• Graduate of a school of nursing, physical or occupational therapy
• Three years’ experience in spine care
• Associate’s, Bachelor’s, or Master’s in Nursing (Preferred)
• Associate’s, Bachelor’s, and Master’s in Physical or Occupational Therapy (Preferred)
• Data analysis (Preferred)
• Program development (Preferred)
• Case management (Preferred)

Licensure, Certification, or Registration Required:

• Current licensure in either:
• State of Florida license as a registered professional nurse (RN) OR Board registration in Physical or Occupational Therapy
• Basic Life Support (BLS) Certification

Job Responsibilities:

• Represents AdventHealth and the Spine Care Team in the community as a ‘Spine Care Ambassador’ planning and coordinating screenings and education in outreach arenas, i.e. speaking engagements, health fairs, corporate affiliation events, etc. Available for TV, print ads, and radio marketing efforts promoting the AdventHealth Spine Care Program.
• Coordinator initiates patient coordination at the patient’s entry point to the system. Reviews scheduled patients to ensure appropriate educational material, patient assessments/questionnaires, and pre-surgical care coordination occurs according to established timelines and algorithms.
• Able to initiate individualized plan of care, identifies needs and develops/modifies plan of care, as needed, in coordination with appropriate spine care team members. Evaluates patients’ developmental level, identifies physical/emotional needs that may affect treatment, and evaluates support system needs to enhance patient care and outcomes. Completes follow-up assessments and evaluations. Investigates questionable situations. Evaluates effectiveness of interventions and medications and facilitates care plan modifications, as needed. Initiates age-appropriate care, as required.
• Coordinates and facilitates an evidence-based practice/performance improvement, interdisciplinary spine care team. Remains current in spine evidence-based practice and new and emerging trends in spine care; shares with the Spine Care Team. Coordinates and conducts spine care clinical training.
• The coordinator coordinates, delegates, provides, and evaluates care of patients, including planning & delivering patient-focused care in partnership with interdisciplinary teams, the patient, and family. Coordinator participates in patient rounds, observes spine patients, and involves nurse manager, physician, physical therapy, or others, as required to address needs of the patient. Coordinator evaluates, monitors, and assists with discharge planning, recommending alternative levels of care, as needed.
• Coordinator prepares equipment, assists assigned nurses, physicians, or therapists, as needed, in the care of the patient. Coordinator ensures that patients’ physical, mental, and spiritual needs are met by coordination with appropriate care team members.
• Gathers, analyzes, and presents Spine Care Program data to Spine Care Program administrative team or other Neuroscience leadership and/or teams to monitor program clinical, operational, quality, and market goals; performs concurrent & retrospective reviews to ensure quality of care.
• Develops appropriate relationships to maximize Spine Care Program effectiveness and facilitates Program problem resolution across departmental, campus and physician/provider lines.
