Work Hours/Shifts: Monday through Thursday 8a – 5p; Friday 8a – 2:30p Job Summary: The overall goal of this position is to improve organizational clinical/operational outcomes for spine related conditions and procedures. The Coordinator coordinates, delegates, provides, and evaluates care of patients, including planning and delivering patient-focused care in partnership with interdisciplinary teams, the patient, and family. The Coordinator helps individuals with spinal conditions navigate the medical system and receive timely and appropriate evaluation, treatment, and education services. The patient needs may range from minimal to complex and may occur in the inpatient or ambulatory arena. The Coordinator is responsible for facilitating a patient and physician friendly experience within the AdventHealth Spine Care Program. The Coordinator assists in planning and conducting community outreach activities, including screenings. The Coordinator assists patients in accessing appropriate resources, as needed. Coordinator may facilitate patient support and/or education groups and strives continually for innovative and creative strategies to affect and improve patient quality of life. Coordinator will gather, record and evaluate program data and to use this data in establishing and evaluating program goals. Coordinator will facilitate, with physician, administrative, and nursing leader(s), an interdisciplinary team to support clinical/operational goals, establish growth/volume/market goals, implement evidence-based protocol, monitor adherence to established patient pathways, etc. The coordinator actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.
