Responsible for provision of pharmaceutical care to patients managed in the Hematology/Oncology clinic with a focus on oral chemotherapy management. Activities include evaluate, monitor and modify drug therapy, provide recommendations/interventions to optimize patient outcomes, minimize safety issues, ensure cost effectiveness as well as provide drug information for medical personnel. The pharmacist will serve as a liaison with the outpatient pharmacy to ensure patients receive the prescribed medication. The pharmacist should be an effective communicator and be able to integrate themselves within the health team and current structure of the pharmacy department. Additionally, the candidate should coordinate pharmacotherapy activities and educational opportunities for department pharmacists and residents in the ambulatory practice setting. Finally, the clinical pharmacy specialist should fulfill requirements associated with hospital and department policies and procedures, JCAHO and other regulatory agencies.
Current pharmacy license to practice in the state of Pennsylvania. Doctor of Pharmacy Degree in Pharmacy from an accredited College of Pharmacy. At least two years of experience in hospital pharmacy practice including precepting students and residents or similar experience in an academic setting preferred. Completion of a Pharmacy Practice Residency and an Oncology Specialty Residency. Candidates should be BCOP or BCOP-eligible
