CRITICAL CARE / EMERGENCY DEPARTMENT CLINICAL STAFF PHARMACIST - FULL TIME ST. LUKE'S THE WOODLANDS HOSPITAL THE WOODLANDS, TX
This position will work 40 hours per week, a combination of day, evening and weekend rotations rotating shifts and will be working in both Adult Intensive Care Unit and the Emergency Department.
As a Comprehensive Pharmacy Services (CPS) Clinical Staff Pharmacist at St. Luke's The Woodlands Hospital you will fulfill daily clinical activities; fill orders with accuracy; check technician-prepared products before release; maintain security of pharmacy and controlled substances; enter orders and patient information with accuracy; aid in the training of new staff; serve as an active member of P&T Committee; and aid the Director in maintaining the pharmacy.
Description: Develop and implement a critical care clinical pharmacy program to improve patient care with focus on providing comprehensive clinical pharmacy services to patients and professional staff, perform clinical activities to ensure evidence-based therapies and guidelines through clinical consultation (e.g., sepsis, stroke, acute coronary syndrome), facilitate medication provision in emergency situations, monitor medication use (e.g., assist with titrating drips), provide drug information support for medical staff, provides patient counseling, ensure hand-off/ continuity of care between ICU/ED and other units, interact with medical staff on drug utilization and cost containment, coordinate with Infectious Disease for Antimicrobial Stewardship Program, develop and implement policies and procedures as a member of P&T Committee, work closely with multidisciplinary team to provide best patient care, precept pharmacy students and interns, oversee ED Medication Reconciliation Pharmacy Technician and provide in-services to medical/nursing staff.

PharmD degree or clinically oriented M.S. degree or BS degree with equivalent experience
ASHP accredited pharmacy practice residency or fellowship or demonstration of equivalent experience preferred
Licensure in good standing in the state of Texas
PGY-2 Emergency Medicine or Critical Care Pharmacy Practice Residency or equivalent Rph ICU work experience
Ability to promote teamwork, build effective relationships, and to lead/facilitate work groups
Strong customer service skills
Knowledge of pharmacy and hospital computer systems and computer-assisted devices
Board Certification (e.g. BCPS), clinical experience in Emergency Medicine and/or ICU preferred
