Serves as the assistant to the Case Management department/team in the coordination of Case Management services and functions. Assist in sending automated referrals for next level of care for patients, communicates with facilities and patient/families to help facilitate discharge plan.

Assist with communicating to patients/families, case manager and nursing staff final discharge time and transportation arrangements.
Responsible for all clerical duties including, but not limited to, copying, faxing, mailing, filing and charting.
