Under the direction of the imaging director and RN director of women's services, the RN navigator will assist cancer patients on their journey through cancer care as an advocate, educator, coordinator and clinical liaison. In conjunction with the Medical Director of the Oncology Program, the Nurse Navigator will: assist in developing protocols, follow the care of each patient, coordinate staff education, and collect data. He/She will participate in the multidisciplinary conference and clinic as well as develop relationships with other service providers.
Associates Degree in Nursing Required
Bachelors of Science in nursing preferred
Computer literate (Microsoft)
Work Experience Requirements
Minimum of 2-3 years of experience in oncology nursing in the inpatient and/or outpatient setting
Experience/knowledge in community health setting/services desirable
Must have a current RN license to practice in the state of Colorado
Certification in oncology nursing preferred.
BLS for healthcare providers certification
BPNC certification preferred (or obtain within 1 year of employment, for Breast Program only)
Position Duties (essential functions, include % of time)
Provide timely and high quality follow-up care and navigation for any patient needing a breast biopsy or diagnosed cancer.
Serve as an essential link between patients and all other care providers.
Facilitate appointments for consults and support services within established service standards, including social workers, dietitians and physicians.
Follow patients throughout the care continuum, including inpatient admissions, and collaborate with inpatient care management resources.
Provides psychosocial support including facilitation of support groups
Coordinates the diagnostic work-up and treatment plan with the team
Maintains consistent and regular communication with the breast cancer patient pre and post operatively
Confirm positive biopsy results with radiologist, and transition care for patients with positive biopsies from radiology to the cancer program
Make phone calls to patient and referring physician for all positive biopsies to ensure patient has follow-up care.
Be available to triage/manage active patient issues for breast surgery practice
Conduct 50 public education events for Summit and surrounding counties
Identifies and provides education to patient/families about current and potential health problems
Provides formal and informal education to other members of the health care team to achieve desired outcomes
Coordinates and participates in community education programs, health fairs and local events
COLLABORATION AND LEADERSHIP-30
Identify and link 750 new patients to mammography or screening services at Summit County Medical Center or surrounding counties
Demonstrates the knowledge, skill and coordination to provide nursing care and guidance to the cancer patient from first contact and throughout follow-up.
Demonstrates advanced clinical and managerial skills contributing to the development, maintenance and change in the delivery system for the care of breast cancer patient.
Works in conjunction with the Director of Oncology Services, Medical Director, surgeons, med oncologists, radiation oncologists, pathologists, radiologists, nurse managers, and clinicians to assure excellence in clinical services.
QUALITY IMPROVEMENT/DOCUMENTATION -10%
Maintains ongoing analysis of the program in conjunction with the Medical Director and Director of Oncology, providing outcome measures to assess efficacy and performance improvement of the program.
Maintains up to date database for providing program reports
Participates in the multidisciplinary Breast conference and documents the treatment plans developed for patients
Participates in professional organizations, attends professional workshops/conferences
