Assumes responsibility and accountability for assessment, diagnosis, intervention and outcome to assure optimal plan of care, as applicable to the role. Performs advanced practice procedures commiserate with training and credentialing. Utilizes the nursing process to promote/restore optimal health and well-being of the patient. Has knowledge of community health concepts, principles of ambulatory clinical nursing care, case management and care coordination. Assumes responsibility and accountability for facilitating, communicating and collaborating with the healthcare team and the patient/ family, utilizes best practice to maximize patient engagement and patient self-management efforts. Promotes the optimal health, well-being and safety of the patient through use of the nursing process and in accordance with patient care standards, guidelines and the State Nurse Practice Act. Demonstrate personal accountability for relationship-based care, organizational mission and core values.
Minimum Education Requirements
Graduate from an Accredited School of Nursing
Master's Degree in Nursing as Nurse Practitioner, from a nationally accredited postgraduate certificate program
Minimum Experience Requirements
One-year experience at an advance practice nurse is preferred
Nursing skills and knowledge base specific to specialty
Current Colorado Registered Nurse Licensure; and listed on the Colorado Advanced Practice Nurse (APN) Registry.
Current BLS from American Heart Association required, ACLS or other certifications may be required depending on practice setting.
Board Certified as an Advanced Practice Registered Nurse, in applicable population foci and/or subspecialty.
Have Prescriptive Authority (RXN) or have Provisional Prescriptive Authority (RXN-P), if applicable for role with associated articulated plan.

Assumes responsibility and accountability for health promotion and/ or maintenance as well as the assessment, diagnosis and management of patient problems, which includes the use and prescription of pharmacologic and non-pharmacologic interventions. *
Provide a scope of services across the health wellness-illness continuum, as defined by nationally recognized role and population-focused competencies as an advanced practice registered nurse. *
Assess patient's physical, psychosocial, cultural and spiritual needs through observation, interview, review of medical records & clinical data. Evaluates patient's response and intervenes in a timely manner to ensure optimal patient outcome. *
Assess patient's ability to participate to the fullest extent by determining presence/ absence of barriers to active participation in the individualized care plan. *

Demonstrate competency in a variety of therapeutic/diagnostic interventions as an advanced practice registered nurse. *
