Provides supervision and assistance to the residents in the activities of daily living, responds to emergency calls and notifies the staff nurse of changes in resident's condition. Assumes personal accountability for organizational mission and core values.
Minimum Education Requirements
High School Diploma or GED or three years of direct patient care
Must be at least 18 year of age for consideration
Minimum Experience Requirements
Ability to read, write, speak, and understand English
Basic computer skills
Previous experience working in a senior community preferred
Current Basic Life Support (BLS) certification from American Heart Association
Position Duties (essential functions denoted with an * )
Assists with safe and appropriate delegated patient care tasks which are directly supervised by a Licensed Practical Nurse or Registered Nurse.*
Must be able to assist residents physically throughout the shift.
Provides quality patient care, including comfort and hygiene measures, preventative skin care, safety measures, ambulation, transport, and nutritional and elimination needs. May perform in a sitter role.*
Promptly responds to patient needs.*
Reports patient status changes and needs to Licensed Practical Nurse or Registered Nurse accurately and in a timely manner.*
Communicates concerns regarding hazards or errors related to patients, families and health care team.*
Maintains thorough and confidential maintenance of patient charts and records.*
Effectively uses time, personnel, equipment and supplies to provide high quality, cost effective patient care. Organizes and prioritizes work to be completed within assigned timeframe to promote consistent patient comfort.
Accurately documents input/output, nutritional intake, personal hygiene, weights and other patient care performed and reports findings promptly to Licensed Practical Nurse or Registered Nurse.*
Ensures that work environment and patient rooms are neat and orderly. Maintains and stocks nursing supplies.*
Uses standards of care, facility policies/procedures and regulatory guidelines to guide work.
Always uses the safety equipment and procedures provided when performing hazardous tasks.*
Promotes resident and staff safety through the use of proper body mechanics, and through frequent rounds on assigned areas.*
Assists with quality improvement, safety and regulatory functions as directed.*
Assists in training other staff, volunteers, and students. Can perform as a preceptor to other Care Partners.*
Consistently demonstrates flexibility and is agreeable to work flexible schedules including weekends, holidays and evening hours as necessary to meet the needs of the patients and facility.
Adapts to rapidly changing technology as evidenced by competency in technology and equipment used in the patient care setting.
Demonstrates effective time management and is proactive in prioritizing workload and meeting productivity requirements.
