Position Summary: We have an exciting opportunity to join our team as a Program Associate.
In this role, the successful candidate The Program Associate is responsible for providing support for care coordination and clinical quality initiatives, contractual requirements, outreach activities, and basic social services promotion to improve patient experience and outcomes. Support may include telephonic, written and/or face-to-face interactions with patients and providers of assigned population(s). This position may also provide assistance in the areas of patient education, health promotion initiatives, and data collection and recording.
Perform other duties to support care coordination, as requested.
Document activities in a care management and/or electronic medical record system according to established standards.
Refer issues and concerns to the appropriate Clinical Care Coordinator or department management.
Promote and support a professional relationship with providers and community resources by participating in education events and outreach opportunities, as needed.
Contact providers to obtain information needed by Clinical Care Coordinators.
Triage patient calls and other inquiries for Clinical Care Coordinators, as needed.
Assist Clinical Care Coordinators with researching appropriate community based resources and social services, and connecting patients to those services.
As directed by a Manager or Clinical Care Coordinator, contact patients to determine if they have seen their PCP and completed, or are in process of completing, their treatment plan or preventive care services.
Assist patients in making appointments with their Primary Care Physician (PCP), Specialist and/or other providers, as indicated and assist with transportation arrangements, as needed.
Provide patients/family with information on preventive and health maintenance services and resources, and on how to access those services while identifying barriers to care.
Conduct telephonic outreach and/or home visits to facilitate preventive screenings, encourage appropriate condition specific services and provide education utilizing approved resources and materials.
Communicate with physician and other provider offices regarding outreach calls and mailings and program questions/issues.
Based upon checklists and information provided regarding potential service needs, communicate with physician offices, collect patient visit/service data, review for evidence of the visit/service, and document the absence or presence of the service in the care coordination system (as appropriate (e.g., communicate about patients needing mammograms).
Conduct outreach and communicate with physician offices, other health care professionals and patients/family.
Minimum Qualifications: To qualify you must have a Associates degree or equivalent is required. At least two years experience in a health related service such as community outreach worker, social work assistant, home health aide, medical assistant, etc.. Understanding of medical terminology. Effective oral, written communication and interpersonal skills including ability to work collaboratively and cooperatively within a team and internal and external customers. Excellent telephonic skills. Strong organizational skills and ability to set priorities. Able to manage a variable workload and to multi-task. Professional demeanor. Appreciation of cultural diversity and sensitivity toward populations served. Able to travel independently and be able to conduct home/ provider office visits, as directed. Must be flexible in a changing environment. Proficient in the use of Microsoft Office.
Preferred Qualifications: College degree or equivalent. Health plan or ambulatory care setting experience. Interest in social work and population health management.
