Clinical Coordinator - Medication Assisted Treatment Program (MAT)

Provides care management for pregnant patients with history of substance abuse. Utilizes system of care processes, clinical expertise, resource management, effective communication, and decision making/problem solving skills to achieve optimal clinical outcomes, patient safety and satisfaction. Must possess knowledge of growth and development appropriate to age group served and incorporate plan to meet needs into plan of care. Provides direct and indirect care as needed, performing patient needs assessments, facilitating referrals, and providing linkages to health, wellness and post-acute resources across the health care continuum. Promotes interdisciplinary collaboration and teamwork to progress the plan of care. Coordinates the delivery of safe, high quality care; identifies areas for study, and participates in and/or supports research. Works in compliance with all State, Federal, and other regulatory requirements. Promotes cost-effective care by minimizing fragmentation, maximizing coordination and facilitating patient/family movement through the health care organization.

Education: Bachelor’s degree required in a human services, counseling, social work , psychiatric rehabilitation , psychology, sociology or vocational rehabilitation as needed to satisfy the current DMAS and licensing standards per the Board of Health Professions. Graduate of an accredited School of Nursing required.
Experience: Minimum of 1 year substance abuse clinical experience in providing direct services to persons with a diagnosis of mental illness or substance abuse; or three years of clinical RN experieince required.
Current Virginia RN license required.
Certified Substance Abuse Counselor (CSAC) required or plan in place to obtain.
AHA BLS-HCP certification required.
De-escalation training must be successfully completed within one year of hire and then maintained going forward.
Other: Effective interpersonal, communication, and team-working skills required. Strong critical thinking, conflict resolution, organization and effective judgement abilities required. Must successfully complete orientation and competency validation requirements. Must be able to meet essential job functions. Designated areas may require knowledge of Medicare/Medicaid and other third party payor regulations..
