Under physician supervision, this position will act as aWellness Visit Nurse (WVN) and take responsibility for coordinating andperforming wellness visits at multiple physician sites. The WVN will preparePersonal Prevention Plans (PPP) on each patient for the provider to review,modify and approve.
Develop schedules for Medicare wellness visits with physician practices.
Educate physician office personnel on wellness visit processes, such as how to determine if patients qualify for these vists, what pre-visit protocols to follow and how to get the health risk assesment to the patient.
Contact patients prior to visit to answer questions and confirm availability for the appointment.
Complete necessary elements of wellness visit in Epic, including flowsheets, individual tests (vision, hearing, etc.) and questionnaires.
Highlight abnormal areas discovered during the wellness visit for provider review.
Use protocols to prepare a Personal Prevention Plan (PPP) for the provider to review, modify and approve.
Provide Personal Prevention Plan and After Visit Summary to the patient with directions for follow-up as recommended by the provider.
Maintain knowledge of Medicare guidelines related to the performance and billing of wellness visits.
Perform other duties that correspond to the overall functions of this position.
Licensed by the Oklahoma Board of Nursing as aLicensed Practical Nurse.
One year of experience in a physician practiceor similar experience.
Current CPR (Healthcare Provider).
