We are seeking a Hospitalist for Continuum of Care, who has a passion for high-acuity, Geriatric Medicine to join our multispecialty physician group that consists of more than 548 primary and specialty care physicians and advanced providers. We have access to leading edge equipment and technology connectivity with hospitals and outpatient centers through EPIC-the premier electronic health record (EHR) system.
We seek a Hospitalist for Continuum of Care to work with a team-oriented group focused on: reducing readmissions, shortening length of stay, controlling acute pain and partnering with ambulatory physicians in the community for continuity aimed at improving the overall health of the people of Southwest Florida. The physician provides attending services to the Skilled Nursing Facility (SNF) and Skilled Nursing Unit (SNU) residents and care management at six local community facilities.

Additional physician responsibilities include oversight of medication management, medication reconciliation, and order management placement for patients assigned to their services; completes H & P, acute visit notes, discharge summary and all associated orders; signs off on death certificates when required. Participates in Quality Improvement Committee activities and provides SNF/SNU nursing staff education to ensure care plans are sustained. Interact with patient's family members as requested to improve support of patient post-discharge. Collaborate with facility's discharge planning teams to help patients successfully transition from SNF rehab services to home functioning or next level of care at their highest level of functioning. The physician also provides supervision and oversight to 4 - 5 advanced providers on the SNF team and Housecalls ARNP team as part of the Continuum of Care. Physicians should be available by phone nights and weekends as needed for emergency requests and will rotate call with the SNF advanced providers. ARNP's in the Continuum of Care are first responders to calls and will escalate calls to the physician for assistance with questions, orders or directives for care.
