Position Summary: We have an exciting opportunity to join our team as a Clinical Pharmacotherapy Spec.
In this role, the successful candidate Participates in the clinical activities of the pharmacy under the direction of the Assistant Director of Clinical Pharmacy.
Develop and maintain a high level of clinical pharmacy practice in all areas of the emergency room and for all patients.
Provides oversight of quality assurance related activities in the emergency room setting to include participation in various meetings; gathering, reviewing and presenting relevant data to the team.
Contributes to the development of all staff: providers, nurses and pharmacists.
Participates in multidisciplinary bedside rounds
Assists in developing and implementing clinical practice guidelines that will enhance efficiency of patient outcomes.
Participates in the gathering of data for review and presenting of pharmacy sponsored medication use reviews.
Responsible for overseeing activities of pharmacy residents and students in the emergency room setting.
Review medication orders written by prescribers while ensuring that those orders are accurate and represent the proper treatment for each patient.
Monitor patients for signs and symptoms of adverse effects to medications administered to a patient in the emergency room.
Participates in codes: stroke, cardiac, malignant hyperthermia.
Participates in the counseling of admitted or discharged patients, as well as their participation and oversight in patient counselling and dispensing of required drug for the Opioid Overdose Prevention Program.
Participates in research.
Serves as a resource for drug information for the ER and other practice settings as needed.
Minimum Qualifications: To qualify you must have a NYS Licensed Pharmacist BS or MS in Pharmacy related field, PharmD. Successful completion of PGY2 Residency in Emergency Medicine 5 years of hospital based Clinical Pharmacy experience.
Qualified candidates must be able to effectively communicate with all levels of the organization.
