Health Program Operations Specialist (E S 10) Functions as a competent Registered Nurse to provide safe and therapeutic care and coordination of services throughout the programs of Structural Heart Services (SHS) and Adult Congenital Heart Disease (ACHD) using the process of assessment, diagnosis, planning, implementation and evaluation. Provides support for area clinical functions.
Job Duties & Performance Standards Expert Practice – Provides expert nursing for the programs of SHS and ACHD Demonstrates knowledge and care of patients in the primary or specialty clinic that the RN is employed, including care and use of specialty specific equipment. Knowledge of medications and their correct administration based on age of the patient and their clinical condition, including the five rights. Prioritizes patient care based on acuity / urgency of patient needs, preference, choices and available resources. Demonstrates knowledge of the principles of growth and development over the life span and the skills necessary to provide age appropriate care to the patient population served. Able to interpret data about the patient's status in order to identify each patient's age specific needs and provide care needed by the patient group. Nursing Process – Systematically assesses the need for and the delivery of nursing services that meet patient care requirements, whether delivered directly or by the other nursing staff. Ability to perform a systematic assessment on pre-procedure structural heart patients (SHS), adult congenital patients (ACHD), and other defined patient populations as needed. Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management. Meets documentation standards and policies. Patient Care Coordination Ability to formulate, coordinate and revise an individualized plan of care as indicated by the patient's response to treatment and evaluate overall plan daily for effectiveness. Treats patients and their families with respect and dignity. Identifies and addresses psychological, cultural, ethnic and religious / spiritual needs of patients and their families. Functions as liaison between administration, patients, physicians and other healthcare providers. Interacts professionally with patient/family and involves patient/family in the formation of the plan of care. Participates in departmental new program development. Consistently follows policies and procedures for providing telephone medical advice to defined patient population including structural heart (SHS), congenital (ACHD), and additional populations as needed. Consistently documents care provided via telephone per UWMC policy. Provides efficient and effective telephone triage for specific patient populations or problems. Patient Safety - Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical / healthcare errors. Complies with specific UWMC policies for safety and infection control. Use of PSN to report events causing or having the potential to cause patient harm. Client Teaching – Integrates patient and family education into the delivery of care. Educational activities are culturally appropriate and are tailored to the patient's and family's ability to comprehend and participate. Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning, family is included in teaching as appropriate as needed. Enabling Operations – Coordinates the care given by others to achieve efficient clinic operations for structural heart (SHS), adult congenital (ACHD), and other defined populations as needed by: Facilitating patient flow and care. Prioritizing and screening patient flow through the program Serving as an interface between staff and physicians. Communicates effectively and appropriately to charge nurse, manager, co-workers, physicians and all members of the health care interdisciplinary team including structural heart, adult congenital, and other defined populations as needed Provides assistance as needed to data extraction for the appropriate registries. Oversight of quality and process improvement Demonstrates an ability to be flexible, organized and function well under stressful situations. Orients new permanent and temporary personnel to the structural heart, adult congenital heart team, and other defined populations as needed in the department. Serves as a preceptor and mentor. Interacts with referring providers as needed Help with coordination of resources for research as it pertains to structural heart programs Assisting as needed with marketing and outreach for structural heart, adult congenital, and other defined populations as needed Adheres to Attendance Standards. Demonstrates knowledge of department specific billing policies and application of their procedures including Facility Only Coding, fee sheet completion and nurse visits. Demonstrates ability to use computer applications required for successful completion of job requirements. Develops protocols to improve care for acute / urgent patient needs. Job-Specific Requirements Regulatory Requirements Current State Registered Nurse Licensure , BSN required. Successful completion of arrhythmia course on monitored nursing units. Language Skills Able to communicate effectively in English, both verbally and in writing. Other Skills Basic Computer Knowledge |
