This position will have an emphasis in the management of patients with solid tumor malignancies. The inpatient oncology pharmacist will work with an academic multidisciplinary team to manage and monitor oncologic emergencies (spinal cord compression, hypercalcemia, superior vena cava syndrome, etc.), intractable nausea or pain, infectious complications, deep vein thrombosis/pulmonary embolism, and cancer therapy adverse effects. A large proportion of patients admitted are managed for immune related toxicities from PD1/PDL1 inhibitors, CTLA4 inhibitors, and other immunotherapies. Furthermore, chemotherapy is administered on this service when the therapy is considered too complex for outpatient administration or the patient’s risk for complications are considered too high for outpatient administration. The pharmacist will ensure the safe and efficacious use of these regimens.
Actively collaborate with the health care team on rounds designing, implementing and monitoring patient-specific pharmacotherapy and chemotherapy plans; interviewing patients to obtain medication histories; counseling patients regarding proper drug dosage, administration, usage, and possible side effects; documenting pharmaceutical care activities
Provide assistance as a member of patient care team ensuring accurate medication use and documentation of medication administration in situations such as cardiac arrests
Assist in the development and implementation of new pharmacy services to support the Michigan Medicine’s goals and objectives
Develop and implement quality improvement and process improvements to improve patient care and pharmacy services
Assure the appropriateness of medication therapy through:
Review of medication orders for clinical appropriateness based on patient specific conditions and concomitant therapies.
Communication with prescribers and providers regarding appropriateness of drug therapy and patient response to therapy.
Assure the appropriateness of medication therapy through: (minority of responsibilities)
Compound and accurately dispense medications, intravenous solutions and related supplies
Formulate, prepare, and maintain quality control of pharmaceutical preparations
Check medication preparation and oversee delegated pharmacy services performed by pharmacy technicians
Pharmacists serve as preceptors for Doctor of Pharmacy students on a variety of practice experiences including Introductory Pharmacy Patient Experiences (IPPEs) and Advanced Pharmacy Practice Experiences (APPEs), and may provide didactic education for pharmacy students
Pharmacists serve as preceptors for pharmacy residents (PGY1 Pharmacy, PGY2 Hematology/Oncology).
May assist in the development and provision of medication-related educational and training programs
May conduct or participate in research, drug use projects, cost reduction/management initiatives, and quality improvement activities consistent with Michigan Medicine’s goals and objectives.
Additional pharmacy and patient care services may include but are not limited to:
Patient education, including but not limited to chemotherapy education, malignancy related symptom management counseling, and anticoagulation education
Doctor of Pharmacy Degree from an accredited College of Pharmacy or equivalent combination of education and experience
Certificate of completion from a PGY-2 Residency in Hematology/Oncology, or an equivalent combination of education, training and experience in Hematology/Oncology.
Current licensure as a registered pharmacist by the State of Michigan Board of Pharmacy and a Michigan Controlled Substance License to practice pharmacy in the State of Michigan, or eligibility to become licensed within 60 days of employment start date

This schedule for this position includes day shift with a weekend and holiday rotation.
This position may be underfilled at a lower classification depending on the qualifications of the selected candidate. (eligibility for Licensure as a Pharmacist by the State of Michigan is necessary.)
This position will report to Pharmacy Manager- Children’s and Women’s Pharmacy Services
Pharmacists provide functional supervision of technical staff, residents, and students working in their area including completion of job assignments and assuring adherence to institutional policies and procedures.
